Ready to be a member of Quill Health? Please send us an email through "contact us" or give us a call for guidance.  Due to influx of enrollments their maybe a waitlist as Dr. Bacala only takes certain number of patients.

 

 

You will be charged a 1 time Enrollment Fee of ($100 + cost of 1st month membership [credit/bank card required]).  Once your information is received, we will be in contact shortly regarding the enrollment process and setting up an appointment. Please review patient contract and forms before enrolling, if possible print and complete them to bring to your 1st appointment.
