Ready to be a member of Quill Health? Please send us an email through "Contact Us" or give us a call for guidance. To maintain the personalized, unhurried care our patients expect, Dr. Bacala accepts a limited number of patients at a time. Depending on current capacity, new patient requests may be placed on a waitlist and reviewed individually — submitting a request does not guarantee enrollment.

You will be charged a one-time enrollment fee of $100 + cost of 1st month membership (credit/bank card required). Once your information is received, we will be in contact shortly regarding the enrollment process and setting up an appointment. Please review the patient contract and forms below before enrolling — if possible, print and complete them to bring to your first appointment.
