Understanding Your Options

TRT and Enclomiphene both raise testosterone, but they work in different ways. Here's a closer look at each:

Common symptoms include low energy, reduced sex drive, difficulty building or keeping muscle, brain fog, and low mood. Low testosterone is confirmed with a blood test — typically a morning total testosterone level, sometimes repeated to confirm before starting treatment.

TRT replaces testosterone directly, typically as a weekly or biweekly injection. Because it supplies testosterone from outside the body, it's a direct, predictable way to bring levels into a healthy range. It can reduce your body's own natural production and fertility while you're on it, which is why it's most often chosen by men who aren't focused on preserving fertility right now.

Enclomiphene is a daily pill that works upstream — it signals your pituitary gland to produce more of your own testosterone, rather than supplying it externally. Because it stimulates your body's natural production instead of replacing it, it generally preserves fertility and testicular function, which makes it a common choice for men who want to keep the option of having children open.

As a general guide: if preserving fertility matters to you, or you'd rather take a daily pill than give yourself injections, Enclomiphene is usually the better fit. If you want the most direct, well-established route and injections don't bother you, TRT is a great option. Erectile Performance: Common Questions

Sildenafil (the active ingredient in Viagra) and tadalafil (the active ingredient in Cialis) are usually prescribed on their own. Our compounded capsule combines both in one dose, so you get sildenafil's stronger on-demand effect together with tadalafil's longer window of effectiveness — a combination you generally can't get at a retail pharmacy.

Sildenafil typically starts working within 30–60 minutes. Tadalafil takes a bit longer to kick in but stays active much longer — up to about 36 hours. The combination is designed to give you both a faster onset and extended coverage.

Not routinely. Your provider will review your blood pressure, medications, and cardiovascular history during your evaluation, and may recommend labs if something in your history warrants a closer look.

Many blood pressure medications are fine to combine with this program, but a few — nitrates and, in some cases, alpha-blockers — are not. Your provider will review everything you're currently taking before prescribing anything.

Let your provider know. Response can vary from person to person, and your dose or approach can often be adjusted at your next follow-up.

Who shouldn't take this

  • Do not use if you take nitrates in any form (including nitroglycerin, or recreational "poppers") — combining them with sildenafil or tadalafil can cause a dangerous drop in blood pressure.
  • Use with caution if you're on an alpha-blocker for blood pressure or an enlarged prostate.
  • Not recommended if you have unstable angina, a recent heart attack or stroke, or severe uncontrolled heart failure.
  • Use with caution with significant liver or kidney impairment, or uncontrolled high or low blood pressure.
  • Tell your provider about any history of sudden vision or hearing loss, or eye conditions like retinitis pigmentosa.
